Does it matter if the waiver form where the applicant sign (and sometimes the recommender too) is outside the signed sealed envelope ? My recommender did not put them in.

I think it should be inside the envelop. that is, what they r trying to establish is that u give the form to the recommender (after signing on it) and then the recommender writes his/her recommendation and returns the recommendation letter to u with the waiver inside.
My suggestion is u should do 1 of 2 things:
1. Contact the admissions school to which u r applying and explain ur situation and ask them if that is possible
2. Ask ur recommender to include the waiver inside the envelop. That is, return the sealed envelop and ask that the waiver be included in the envelop before it being returned to u.

thanks... I hope it won't matter... the letter being sealed means I couldn't tamper with it and the waiver is your own (signed legal) declaration, which is also not mandatory, so...
