I would like to know if NYU and Harvard require or provide a form to accompany your letters of recommendation?

I think that Columbia doesn t require any form just the letter...is just an exception?

NYU does provide.

Although I would think most schools provide some sort of recommendation form, none of them actually require that you use it. A normal letter is fine. If it's sent physically it should be on official letterhead though.
