Ok guys,
Has anyone actually thought about this? Who makes the decision about a student? The dean of graduate studies? A committee? Does anyone know for sure?

That depends on the school. Usually the applications are read by more than one person.
NYU's Dean explained that each application is read by three people. If you have a unanimous response (positive or negative) from them, you are either admitted or rejected. If not, which he said are the most cases, the applications are directed to a committee that will make such decision.
