I am about to finish my CLS application however after double checking It seems as if I have to send a supporting document called the Certification Form by mail. Does anyone know what this form is and where I can find it?? (I haven't seen it on their web site)!! PLEASE HELP!!!

don't worry, the certification form is a form that will appear on the website once you've completed your application, i.e. once you've sent it online
then the certification form will become available for download

Oh, BTW, my professor sent the recommendation letter directly to the University. Is there any problem in doing this?
