I have to fill my IELTS form and give details of Ghent University, however I couldn't find any information from the university's website.

These fields have to be filled:

Name of person/department
Name of institution
File/Case number (I don't have any idea what to write here)
Adress

I would be very glad if someone who filled the same form can help me. Additionally, I would be very glad to meet with people from 2012-2013 class.
