Not sure if I understand the question entirely, but I'll try...
You need the form from LSAC to accompany all transcripts. So if that is your question, yes.

Check with the Graduate Admissions Office to be sure, but I asked them this very question and they said that the Registrar's Certification Form was just for the law school. Undergrad transcripts do not need it. But, give them a quick call--I'd hate to steer you wrong.
