I WANT TO CHANGE MY NAME ON LSAC HELP

I didn't know that we were supposed to put middle names on LSAC so my full name on LSAC doesn't match my ID, transcript, etc. Has anyone ever had this experience and mail to LSAC?

Hey there! Well, i had to fax them my request in order to have my date of birth changed.
So you could try that.

What did you fax? Was there a form that you completed? And i’m also wondering if we could email it to LSAC considering I don’t have any access to fax

A written request asking the relevant change. Of course, i did provide my lsac number etc.
Well, you could always email them and ask them!

Oh so there's no specific form? Also how long did it take for the data to change? Sorry for all these questions I'm just very worried and anxious
