I didn't know that we were supposed to put middle names on LSAC so my full name on LSAC doesn't match my ID, transcript, etc. Has anyone ever had this experience and mail to LSAC?

A written request asking the relevant change. Of course, i did provide my lsac number etc.
Well, you could always email them and ask them!

Oh so there's no specific form? Also how long did it take for the data to change? Sorry for all these questions I'm just very worried and anxious

You should not worry my friend, you have plenty of time until the application deadlines.
It took them 2 working days.
